OK,
Let's try to sum up ideas, considering that everybody thinks differently.

1. Identity
At the time being the identity is check through a mandatory mail box.
This is not to check who has answered what, because this email adress could be wrong or a yahoo one which is in a way anaonymous.
So the only aim of this check is to be able to, for example, answer the question of somebody who wants to be sure that his form arrived.
We have understood this questionnaire as a seek for ideas not a secured vote. If people want to script this page, considering that answers will be humanly treated, I think people behind will see the thing. By the way there is no technical mean to prevent scripting this kind of page except by a one answer for one man process, secured, like the voting process.

2. Bugs in the Questionnaire
Considering that a lot of people were at the ICANN meeting, we didn't have much reaction on the actual state of the questionnaire. All your remarks will be quickestly possible taken into account.
	- multiline text boxes : considering that the form produces in output all answers one after another, I am afraid that the possibility for people to put carriage return will drastically complicate the task of people who will have to sort the answers....
